Behaviour Change Specialists
Behaviour Change Specialists will work to a place-based model. This will require close collaborative working with communities, other organisations and partners delivering services in specific communities.
Behaviour Change Specialists will utilise local knowledge to provide the most effective service, tailored to the needs of their local population.
What You Need to Know
- Our Behaviour Change Specialists will work with individuals aged 16+ on a 1-2-1 basis.
- Our specialists will work with individuals from disadvantaged communities and certain priority population groups who are at a higher risk of engaging in unhealthy behaviours. This is to encourage and enable those individuals to improve their health.
- Interventions are available face-to-face or remotely via video or phone to ensure maximum accessibility. In-person appointments are offered at various community and primary care sites across the borough, with evening and weekend options tailored to individual needs.
- Our specialists will offer a maximum of six sessions (not including follow-up), within a maximum of one year, unless specified differently within the specific intervention information.
- Our specialists assess each individual’s readiness to change, identifying barriers and current habits. Using these insights, they provide personalised guidance to empower sustainable lifestyle shifts. Finally, specialists and individuals co-produce a tailored action plan with specific goals for a set timeframe.
- Sessions are typically held on a 1-2-1 basis, but groups will be explored depending on need. Innovative solutions are required to help provide support for the ongoing maintenance of behaviour change. This will include community or peer support along with digital innovations.
- Our specialist will follow up with all individuals 6 and 12 months post completion, and individuals who have not maintained their behaviour change will be offered a referral to further appropriate support.
