Recently diagnosed with type 2 diabetes?

A type 2 diabetes diagnosis can be a life-changing and scary moment. Having a good support network can help you to come to terms with your diagnosis, all of the emotions that come with it and how you can manage it.

With the right tools and mindset, you can manage your condition and live a happy, healthy life which is what our Health Trainers aim to help you to achieve.

What support can you get from a Diabetes Health Trainer?

With this service, you will be assigned your own Health Trainer, specialising in those living with type 2 diabetes, who will help you to understand more about type 2 diabetes and the impact it has on your own lifestyle so you can work together to set realistic health goals.

Over 6 sessions of support scheduled at a convenient time for you, you and your trainer will work in partnership and develop a bespoke plan that is tailored to your individual lifestyle and goals to support you in making healthy lifestyle changes.

Our Health Trainers can help you with many elements of your health such as:

  • Managing diabetes
  • Healthy eating for diabetes
  • Maintaining a healthy weight
  • Increasing physical activity

Who can get healthy with us

Please note that this is a pilot programme and at this stage, only patients registered with the 5 early adopter GP practice sites across Cambridgeshire & Peterborough can join the Diabetes Health Trainer service.

These are:

  • Boroughbury Medical Centre & Paston
  • Cathedral Medical Centre (Ely), St George’s Medical Centre & St Mary’s Surgery
  • Staploe Medical Centre, Burwell Surgery & Haddenham Surgery
  • Hampton Health & Octagon Medical Practice
  • Clarkson Surgery, North Brink Practice, Parson Drove Surgery & Trinity Surgery

If you are registered with one of the above practices and you have been diagnosed with type 2 diabetes within the last 12 months, please discuss the service with your GP who will be able to be refer you to Healthy You for support.
